STREET OUTREACH & ENGAGEMENT Street Outreach & Engagement serves

and engages the most vulnerable

homeless population in the City of

Fontana. We focus on the chronically

homeless, those who have been on

the street one year or longer and who

suffer from mental illness, substance

abuse, physical disabilities and extensive

trauma. We go into encampments,

parks, water channels and fields in order

to connect homeless individuals and

families with services such as health

care, documentation, income sources,

job employment, shelter and housing.

In collaboration with the Fontana

Police Department and other agencies,

we have engaged with hundreds of

homeless individuals and families and

have successfully bridged them to

shelter, housing and other services.
